Health needs

If you, or a member of your household, are affected by difficulties relating to illness or disability and your current home is therefore unsuitable; please complete the Medical Assessment Form at the back of your Housing Application.

A Medical Assessment Form will be required to be completed for each individual household member with medical needs.

Any medical form received will be assessed by an independent medical examiner.
